Register as a patient

Quote / Testimonial:
We are currently welcoming new patients to register with our surgery, including those who live outside of our practice boundary. Please see our map below for further details.

Please note, if you are located outside of the boundary then we will not be able to offer home visits, but you are still very welcome to register with us.

Registering Online

We are using a new online service called Register with a GP surgery that makes it easy to register with Cumberland House Surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

Registering in-person:

Simply download, print out and complete a new patient registration form at home and bring it into the surgery. We have printed copies here if you don’t have a printer. Once this has been signed, your details will be added onto our system so you can book appointments and contact our practice.

Temporary registration when you need emergency treatment

If you are ill while away from home or you’re not registered with a doctor, we can provide emergency treatment for 14 days. Following this you’ll need to register on either a temporary or permanent patient.

You can be a temporary patient for up to three months, and you can remain registered with your permanent GP during this time. After three months you will have to re-register as a temporary or permanent patient.

Practices don’t have to accept you as a temporary patient, although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
