New Patient Registration
We provide services over the whole of Macclesfield and the surrounding area within a three-mile radius. New patients are free to register providing they live within the practice boundary.
We ask all new patients to fill in a registration form and to complete a new patient questionnaire. Both of these forms are available to download from the above "Registering Online" tab.
All patients who request to register at the practice are required to provide two forms of identification, one must show your current address, and one must show your date of birth, eg a utility bill (no older than 3 months) and a driving licence or passport would be ideal.
It is important that you provide us with your previous doctor’s name and address.
A practice leaflet is provided which includes all the details about the surgery and service we provide.
Once we receive the completed forms we will arrange for a new patient appointment with our Health Care Assistant, where a urine sample is needed.
Your registration details will then be added to our computer system, enabling you to contact the surgery and book appointments.
Medical treatment is available from the date of registration. Please contact reception for further information.
