Registration

New patients are free to register providing they live within the practice boundary.

We ask all new patients to fill in a registration form and to complete a new patient questionnaire. It is important that you provide us with your previous doctor’s name and address. A practice leaflet is provided which includes all the details about the surgery and service we provide. Once we receive the completed forms we will arrange for a new patient appointment with one of our nurses, where a urine sample is needed. Your registration details will then be added to our computer system, enabling you to contact the surgery and book appointments.
